GDPR - Consent to the processing of personal data
granted in accordance with the provisions of Article 7 of Regulation (EU) 2016/679 of the European Parliament and of the Council on the protection of individuals with regard to the processing of personal data and on the free movement of such data and repealing Directive 95/46 / EC ("GDPR") and no. 40/1964 Coll. Civil Code as amended (hereinafter referred to as "consent").
As the legal representative of the data subject, I hereby freely and seriously give my voluntary, informed and express consent to the processing of personal data:
my child to the extent: name and surname, video, audio and audio-visual recordings,
my person in the range: name and surname, video, audio and audio-visual recordings,
non-profit organization Kids Chamber of Crafts and Commerce, no, with its registered office at: Rázusova 13, Košice, 04001, Slovesnko (hereinafter referred to as the “Operator”), which the Operator may process for the purpose of presentation and promotion of the Operator and its activities, www.kidcham.com and social networks as well as the organization of events organized by him.
The duration of processing and storage of personal data is determined for the entire period of existence of the Operator, with the current duration of the purpose of processing personal data, which is also the presentation and promotion of the Operator.
Terms of personal data processing
I give this consent in accordance with the provisions of Article 7 of the GDPR and Act no. 40/1964 Coll. The Civil Code, as amended, and I acknowledge that the consent is valid until its possible revocation, for the entire duration of the purpose of data processing for which the consent was granted.
After the expiration of the consent period, the personal data will not be subject to further processing.
I declare that in accordance with the provisions of Article 12 of the GDPR, the Operator has formulated to me in a transparent and comprehensible form the information specified in the provisions of Art. 13 and 14 of the GDPR and all notifications under Art. 15 to 22 and Art. 34 GDPR.
I declare that the Operator has provided me with information on the right:
request from the Operator access to personal data concerning the Affected Person pursuant to the provisions of Art. 15 GDPR,
to correct or delete or limit the processing of personal data in accordance with the provisions of Art. 16, 17 and 18 GDPR,
object to the processing of personal data pursuant to the provisions of Art. 21 GDPR,
to object to automated individual decision-making, including profiling, and to the right that the decision in question does not apply to me, in the event that the Operator processes personal data exclusively by automated decision-making, including profiling according to the provisions of Art. 22 GDPR,
for the portability of personal data pursuant to the provisions of Art. 20 GDPR,
to submit a complaint or a proposal to initiate proceedings to the supervisory body, which is the Office for Personal Data Protection, if I am directly affected by my rights under the provisions of Section 100 of the Personal Data Protection Act.

I acknowledge that the Operator is in accordance with the provisions of Art. 34 GDPR, obliged to notify me without undue delay of a breach of personal data protection, if such breach of personal data protection may lead to a high risk for my rights.
In conclusion, I declare that the Operator has provided me with information about the right to withdraw my consent to the processing of personal data at any time in accordance with the provisions of Art. 7 pt. 3 GDPR.
At the same time, I declare that I have been acquainted with the activities for the purpose of which this consent was issued and with the method of processing my personal data.
